The evidence, country by country

The protocol that the world copied was written in Amsterdam. The first health authorities to read the evidence and change course were Finnish and Swedish. Much of that material has never been translated. These dossiers set out what each European country's own documents actually say, with dates and links.

The Dutch protocol: origins and follow-up data

Puberty blockers did not originate as a treatment for adolescent gender distress. Gonadotrophin-releasing hormone agonists, usually abbreviated to GnRH agonists or GnRHa, were already used in paediatric endocrinology to suppress unusually early puberty, and in other branches of medicine for hormone-sensitive conditions. The Dutch innovation was to apply these drugs to adolescents diagnosed with gender identity disorder, later termed gender dysphoria, as part of a staged pathway towards medical transition.

Sweden: Karolinska, SBU and the 2022 guidelines

Sweden’s change was not the result of a new Act of Parliament banning medical transition for minors. It was a sequence of clinical and national evidence-policy decisions. The first was local: Astrid Lindgren Children’s Hospital, part of Karolinska University Hospital in Stockholm, altered its practice in spring 2021. The second was national: Socialstyrelsen, Sweden’s National Board of Health and Welfare, revised its recommendations for under-18s in 2022 after commissioning an evidence review from SBU, the Swedish Agency for Health Technology Assessment and Assessment of Social Services.

Finland: COHERE and the 2020 recommendation

Finland’s Council for Choices in Health Care, known in Finnish as PALKO and in English as COHERE Finland, operates in connection with the Ministry of Social Affairs and Health. Its role is to recommend which treatments belong within publicly funded healthcare. Its recommendation, Medical treatment methods for dysphoria associated with variations in gender identity in minors , was adopted on 11 June 2020 and published in summary form five days later (COHERE Finland, 2020).

Norway: the UKOM report and the 2023 warning

Norway’s 2023 intervention in the debate over medical transition for minors came not from a political party or a campaigning organisation, but from Ukom, the Norwegian Healthcare Investigation Board. Ukom is an independent public body established under the Act of 16 June 2017 on the Norwegian Healthcare Investigation Board. Its task is to investigate serious incidents and serious conditions in health and care services in order to improve patient safety through learning and prevention; it does not decide criminal or civil liability (Act on the Norwegian Healthcare Investigation Board, 2017).

Germany: the S2k guideline and the self-determination law

Germany’s position is frequently presented as a direct answer to the Nordic reappraisals of paediatric gender medicine. That is only partly accurate. Germany has two distinct policy tracks: a clinical guideline for assessment and treatment, and a civil-status law on the legal recording of sex and names. The two are politically connected in public debate, but legally separate.

France: the Académie de Médecine and the Senate report

France’s debate on medical pathways for gender-distressed minors has often been described as a dispute between “affirmative” care and a more exploratory clinical approach. The national record is more complicated than that shorthand suggests. France has had specialist hospital consultations, but no final, nationwide clinical guideline specifically governing the care of minors. The debate is usually conducted in English about English-speaking countries, which means the most consequential European documents are quoted second-hand or not at all. Sweden's systematic review, Finland's recommendation, Norway's report and the Dutch follow-up studies are all public.
